Urban art for Hope has adopted a system, socio-economic empowerment for healthier and socio-economically empowered communities. All our staff work through our inter-disciplinary teams to address the Socio-Economic determinant’s of health of our target populations.

OUR MODELS

Health Promotion

Health promotion is the process of enabling people to increase control over & improve their health. To reach a state of complete physical mental & social wellbeing, an individual or must be able to identify of realize aspirations to satisfy needs and change on cope with the environment. Health is therefore see as a resource for everyday life, not the objective of living. Health a positive concept emphasizing social & personal resources as well as physical capabilities. There health promotion is not just the responsibility of the health sector, but goes beyond healthy lifestyle to wellbeing.

Prerequisites for Health

The fundamental conditions and resources for health are peace, shelter, education, food, income, a stable ecosystem, sustainable resources, social justice & equity. Improvement these basic prerequisites.

Advocate

Good health is a major resource for social, economic and personal development &an important dimension of quality of life. Political, economic, social, cultural, environment, behavioral & biological factors can all favor health or be harmful to it.
Health promotional action aims at a marking these conditions favorable through advocacy for health.

Enable

Health promotion focuses on achieving equity in health. Health promotion action aims at reducing differences in current health status & ensuring equal opportunities resources to enable all people to achieve their fullest health potential. This included a secure foundation in a supportive environment, access to information, life skills & opportunities for making healthy choices. People cannot achieve their fullest health potential unless they are able to take control of those things which determine their health. This must apply equally on all genders.

Mediate

The prerequisite s & respected for health cannot be ensure by the health sector alone. More important, health promotion demands coordinated action by all the like minded parties involve: by government, by health & other social economic sectors, by nongovernmental and voluntary organization, by local authorities, by industry and by the media. All round life individuals, families and communities are involved. Professional and social groups and health personnel the major responsibilities to mediate between differing interests in the society for the pursuit of health.
Health promotion strategies and programs should be adapted to the local needs and possibilities of individual’s counties and regions to take into account differing social, cultural and economic systems.

  • Target population needs –Based

Urban at for Hope strive to improve the wellbeing and health of our target population within our catchment area.

We analyze the needs of our diverse population groups and develop action plan to handle them.

  • Community development oriented

We work with community members to develop solutions that support healthy living that’s through civic engagement campaigns that enable the community members to actively participate in public policy decision making, affordable housing and community income generating projects i.e in line to the SDG of the UN.

Driven and Based on determinants of health. For instance, in serving people facing SOCIO-Economic challenges. We provide additional support like transit tickets to the various health facilities in getting health appointments, counseling on how to secure employment on access to additional social services and peer support groups. We also advocate for healthier public policies that enable our target population to access a healthy environment and afford other basic necessities of life.

  • Integrated and interprosessionaly coordinated teams

Our professional teams comprise of members in interprosessional terms extending beyond clinical providers to include health promotes social workers, outreach and community development workers. The respective teams collaborate with a wide range of primary code providers as well as other parts of the health and social service system.

  • Promoting a culturally safe and anti-oppressive practices.

Urban Art for Hope recognizes many populations and well- being.as such we ensure our staff undergo capacity building on issues of anti-oppression and culturally safe practices.

  • Accessible, accountable and efficient

Our priority is in providing appropriate access to everyone, no matter who they are or where they live. Accessibility requires breaking down all social, cultural, linguistics, physical, social, economic, legal and geographic barriers that prevent people from accessing health services. On the other hand community governance also ensures we are accountable to the communities and their founders. In addition we develop and apply reporting indicators so founders can track their performance with respect to effectiveness and efficiency.

  • Community governance

Through wide range of engagement processes, we constantly reorient services to meet communities changing needs. This approach is strengthening by another defining feature of the model, community governance. Community members set the strategic direction of the organization serving them.

Sexual Health Clinic

Offered in partnership with private health facilities, and government health facilities which provides birth control counseling, Sexually, Transmitted Infections (STI) testing, pregnancy counseling and referrals.
