Urban at for Hope strive to improve the wellbeing and health of our target population within our catchment area.
We analyze the needs of our diverse population groups and develop action plan to handle them.
- Community development oriented
We work with community members to develop solutions that support healthy living that’s through civic engagement campaigns that enable the community members to actively participate in public policy decision making, affordable housing and community income generating projects i.e in line to the SDG of the UN.
Driven and Based on determinants of health. For instance, in serving people facing SOCIO-Economic challenges. We provide additional support like transit tickets to the various health facilities in getting health appointments, counseling on how to secure employment on access to additional social services and peer support groups. We also advocate for healthier public policies that enable our target population to access a healthy environment and afford other basic necessities of life.
- Integrated and interprosessionaly coordinated teams
Our professional teams comprise of members in interprosessional terms extending beyond clinical providers to include health promotes social workers, outreach and community development workers. The respective teams collaborate with a wide range of primary code providers as well as other parts of the health and social service system.
- Promoting a culturally safe and anti-oppressive practices.
Urban Art for Hope recognizes many populations and well- being.as such we ensure our staff undergo capacity building on issues of anti-oppression and culturally safe practices.
- Accessible, accountable and efficient
Our priority is in providing appropriate access to everyone, no matter who they are or where they live. Accessibility requires breaking down all social, cultural, linguistics, physical, social, economic, legal and geographic barriers that prevent people from accessing health services. On the other hand community governance also ensures we are accountable to the communities and their founders. In addition we develop and apply reporting indicators so founders can track their performance with respect to effectiveness and efficiency.
Through wide range of engagement processes, we constantly reorient services to meet communities changing needs. This approach is strengthening by another defining feature of the model, community governance. Community members set the strategic direction of the organization serving them.
